RPM to streamline CHF program

Dunmore, Pa.-based Traditional Home Health and Hospice is beefing up its Congestive Heart Failure (CHF) patient management program with an in-home patient vital signs monitoring system. The agency, which provides home health and hospice services to patients, has implemented the Electronic House Call system from Berkeley Heights, N.J.-based ExpressMD Solutions.

The remote patient monitoring system features a Web application that promises to streamline the practitioner's job anywhere they have Internet access, or a Windows mobile communication device. Officials at Traditional Home Health and Hospice expect their new RPM system to significantly enhance the agency's ability to manage CHF patient care programs.

Electronic House Call transmits patient vital signs data on a daily basis through the Internet to patients' licensed providers, so they can monitor and adjust patient care accordingly.

In addition to home health and hospice services, Traditional offers its' patients nursing, physical therapy, speech therapy, medical social services, dietary counseling and other services in the home setting.
